In order to ensure everyone's safety, we kindly ask our clients to review our guidelines, read and sign the intake/consent form agreement prior to their visit. Thank you for your consideration.



  • Please review all the guidelines, read and sign the consent agreement. 

  • Starting January 3rd, 2022, proof of vaccination is required in compliance with CCDPH.

  • Please do a self-health check before your session and do not come if you have COVID-like symptoms.

  • I will do a daily self-check, wear a mask, and prepare/sterilize the room. I am fully vaccinated.

  • Pillowcases and sheets will be freshly washed prior to your visit. 

  • The air purifier unit will run before the session for air circulation.

  • Please wear your mask the entire visit.

  • You may bring your own water bottle if you wish.

  • Please be conscious of others and maintain safe distances at all times.



  • I will welcome you as you walk in.

  • During the session, I will avoid direct skin contact.

  • Sessions will be 60 minutes unless you request a shorter session. 

  • During your session, if you need to stop and break for any reason, we will do that.  


  • We will talk briefly together or follow up via email if this is your preference.

  • Please maintain safe distances as you leave the building.



  • I understand that the novel Coronavirus (COVID-19) has been declared a global pandemic by the World Health Organization (WHO). I further understand that COVID-19 is extremely contagious and may be contracted from various sources. I understand COVID-19 has an incubation period during which carriers of the virus may not show symptoms and still be contagious.

  • I understand that I am the decision-maker for my health care. To the best of their ability, my practitioner will provide me with information to assist me in making informed choices. This process is often referred to as “informed consent” and involves my understanding and agreement regarding recommended care, and the benefits and risks associated with the provision of health care during a pandemic. Given the current limitations of COVID-19 virus testing, I understand determining who is infected with COVID-19 is exceptionally difficult.

  • I understand that preventative measures and intensified sanitation protocols intended to reduce the spread of COVID-19 will be implemented. However, because this work involves close physical proximity over an extended period of time in a closed space, there may be an elevated risk of any disease transmission, including COVID-19. I hereby acknowledge and assume the risk of my actions and give my express permission to you and the staff at your offices to proceed with providing care.
